Once you have an Android device (version 7.29.3 and above), you're able to get your PUK from the MyDigicel app.
Follow the steps below:
Step 1. Log in to the MyDigicel app & select the menu icon.
Step 2. Select "Settings".
Step 3. Select 'Request PUK'.
Step 4. Enter your phone number & your password.
Step 5. Your PUK will be displayed.
